HEALTH FORMS

 HEALTH INFORMATION/RESOURCES

Ceres Unified School District Nurses are dedicated to the health, safety and welfare of each student. CUSD nurses provide annual screenings to students. For more information, please contact your child's school. Parents can opt out of screenings or other health services by submitting a written statement to the school each year.

We encourage you to keep your children home if they are sick and contact your health care provider if they develop flu-like symptoms.  Symptoms include fever, cough, sore throat, body aches, headache, chills and fatigue. Remind your children to cough or sneeze into their elbow, not their hands, and remind them to wash their hands frequently with soap and water.
